Offers a variety of services and resources to help family caregivers to provide good care for older relatives, while also taking care of themselves.

Provide free casework services to Baltimore County seniors, age 60 and over, to help them remain in their own homes and in the community. Develop care plans to meet the clients' needs using available services and funding sources. Clients are monitored and reassessed regularly.

These services are provided through the Bureau of Community Health Services primarily for disabled adults and senior citizens at risk for long term care placement.

• Adult Evaluation and Review Services (AERS): A state-mandated program involving the assessment of at-risk functionally and chronically disabled adults to determine home and communtiy-based services that will meet their needs and enable them to avoid institutionalization in state psychiatric facilities, nursing homes, and other long term care facilities. • Case Managment Program: Designed to assist clients at-risk of needing long term care services to remain in the community. Staffed by nurses and social workers who provide assessment, care plan development, service coordination and monitoring of those services. Any Baltimore County resident needing assistance to coordinate services enabling them to remain in a non-institutional setting may be referred for case management services. There is no cost for the assessment and case management services. Coordinated services are on a fee-for-service basis or publicly-funded depending upon eligibility and availability.

• Statewide Evaluation Planning Services (STEPS) Program: Provides comprehensive evaluation and assists with care plans for individuals requiring long term care. Evaluations are conducted to determine whether community-based services could appropriately substitute for nursing facility care. Assessment is open to any Baltimore County resident in need of a long term care assessment or considering nursing home placement. NO cost to clients seeking an assessment. Baltimore County is reimbursed by the State of Maryland for STEPS eligible clients.

Client must be homebound, have a need for skilled services, and be under the care of a physician to be eligible. Program is licensed as a Medicare certified Home Health agency.

• Home Health Services: Provides intermittent skilled nursing, physical therapy, occupational therapy, speech therapy, medical social service, and home health aide to homebound Baltimore County residents under the direction of their physician.

• Hospital Support Services: Nurses also provide no cost discharge planning assistance to Baltimore County residents in conjunction with hospital staff and physicians to provide a smooth transition from hospital to home and implementation of needed services and resources succh as home health nursing and physical therapy.  The service is offered to clients who are being discharged from Baltimore County hosiptals.

In-home aides help qualified elderly, chronically ill, or disabled persons remain in their homes by helping them with some simple daily activities such as bathing, dressing, feeding, grooming, and toileting. This program is designed for Baltimore County residents, medically eligible Medical Assistance / Medicaid recipients, living in a "home" and under the care of a physician. Services are provided in the recipients' home by an approved Personal Care Provider under the supervision of a Registered Nurse. Cost of the program is covered by the Maryland Medical Assistance Program.

Provides crisis intervention to vulnerable adults living in Baltimore County aged 18 and older with no willing or capable caregiver available when needed, those at risk of institutional placement, and those at risk of abuse or neglect will be given preference for service, using a ranking scale. Designed to help those who have functional disabilities and need assistance with personal care, chores, and/or activities of daily living, in order to remain in their own homes.

A statewide program for eligible adults age 18 to 64 with long-term or severe physical disabilities that provides financial reimbursement to supplement individuals' costs of attendant care. Attendant care services may include in-home assistance with personal care, chore services, and transportation. Reimbursement is based on annual income, family size, and Program funding.
